Our Work
GP Practices are facing more challenges and greater demand than ever before, our aim as a Primary Care Network is to offer support to our practices to ensure that all people are at home living a happy and healthy life.
Working closely with City Health Bradford GP Federation we provide oversight and support to ARRS (Additional Roles Reimbursement Scheme) Staff, who are an integral part of the PCN Practices workforce.
Enhanced Access Service
Horton PCN Provides an in hours Enhanced Access service that is available to patients aged 3-16 every weekday between 3:30 – 5:30 PM, this provision is specifically catered to younger patients who have been sent home from school unwell to support the PCN practices with their capacity in the afternoon. These clinics are held at Little Horton Lane Medical Centre.
Phlebotomy (Blood taking) appointments are also available for patients with long term conditions such as diabetes as well as wider health inequalities clinics that are catered to the local patient’s needs.
Appointments are also available to patients out of hours between 6 – 9 PM with, GPs, Nurses, Health Care Assistants and Physiotherapists.
Student Placements
Horton PCN supports its practices with the placement of Physician Associate students from the University of Bradford.
